Facility Evaluation Report
While at the facility conducting investigation of a complaint (Complaint Control # 15-AS-20250710162549), Licensing Program Analyst (LPA) Delmundo learned that three residents were sent out to the hospital, two in June 2025 and the other one on July 2025, and the facility did not submit Unusual Incident Reports.
Deficiency is cited from Title 22 California Code of Regulations and listed on 809D. Failure to submit proof of correction by plan of correction due date may result in civil penalty.
LPA called and left message on Roselyn Chand's (administrator) voicemail regarding the above. LPA also discussed with Joan Acob, staff, the deficiency and plan and proof of correction.
Exit interview conducted. Appeal Rights, LIC9098 Proof of Correction form, and copy of this report provided.
