Facility Evaluation Report

On 07/08/2026 at 8:00 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct a 1-Year Annual Required inspection. LPA met with Backup administrator, Nelson Galdo, and explained the purpose of the visit. LPA called and spoke with Regional Director (RD) Lilibeth Lopez. RD arrived at a later time. The facility’s fire clearance was approved for six ambulatory-only.

LPA toured the facility, including but not limited to the bedrooms, bathrooms, kitchen, common area, and backyard. The facility has 5 bedrooms, all of which are occupied by clients. All indoor and outdoor passageways are kept free of obstructions. A comfortable temperature for clients is maintained at 71 degrees Fahrenheit. LPA observed that the lighting in all rooms is adequate for clients' comfort and safety. The hot water temperature in the shared clients’ bathroom was measured at 112.2 degrees Fahrenheit. All toilets, handwashing, and bathing facilities are safe, sanitary, and in good working order. There is a minimum of one week's supply of nonperishable food and a 2-day supply of perishable food.
