The inspector’s account

On 07/22/2025 at 8:35 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with staff, Nelson Galdo, and explained the purpose of the visit. Administrator came shortly after. Administrator certificate is current. The facility’s fire clearance was approved for six ambulatory only.

LPA toured the facility including inside and out but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 5 total bedrooms of which all bedrooms are occupied by the clients. All outdoor and indoor passageways are kept free of obstruction. A comfortable temperature for clients is maintained at 71-degree Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 110.4-degree Fahrenheit. All toilets, hand washing and bathing are safe, sanitary and in operating condition. There is a minimum of one week supply of nonperishable and a 2-day perishables food supply.

Smoke detectors and carbon monoxide were in operating condition during the visit. Fire extinguisher was last serviced on 10/17/2024. Earthquake drill was last conducted on 05/04/2025. First aid kit was observed to be complete.

At 9:19 AM, 6 of the clients’ records were reviewed. At 9:53 AM, 4 staff records were reviewed. Staff have current first aid training and are associated with the facility. LPA reviewed clients' P&I money with log. LPA reviewed two samples of clients’ medications.
