Facility Evaluation Report

At 1:35 p.m., on this day, April 28, 2026, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA was granted entry by staff, Teresita Aquino, and informed the reason for visit. LPA called and spoke over the phone with Regional Director (RD) Lilibeth Lopez. Mildred Gonzales, administrator (ADM), arrived at 1:59 pm.

LPA toured the facility inside out with Teresita Aquino. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, front, side and backyard. There's supplies of 7 days of non-perishables food.

Facility has carbon monoxide and smoke detectors that were tested and observed in operating condition. Fire extinguisher checked, observed fully charge with tag showed serviced October 17, 2025. Hot water temperature in the ensuite bathroom was tested and measured at 105 degrees Fahrenheit. Disaster drills conducted monthly and records showed last conducted April 5, 2026.

LPA reviewed 2 staff records. Currently, facility does not have residents.
