The inspector’s account

At 1:55 p.m., on this day, April 9, 2024, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA was granted entry by staff, Teresita Aquino, and informed the reason for visit. LPA called and spoke over the phone with Regional Director (RD) Lilibeth Lopez. Mildred Gonzales, administrator (ADM), arrived at 2:08 p.m. RD arrived at about 2:20 p.m.

LPA started the inspection with Teresita Aquino and continued with ADM. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, garage, front, side and backyard. Food supplies were observed good for 2 days of perishables and 7 days of non-perishables. Central storage for medications was observed locked.

Facility has carbon monoxide and smoke detectors that were tested and observed in operating condition. Facility conducts fire and earthquake drills at least quarterly and records showed last conducted March 3, 2025 and March 1, 2025 respectively. Fire extinguisher checked, observed fully charge with tag showed serviced October 17, 2024. Hot water temperature in the common bathroom was tested and measured at 117.5 degrees Fahrenheit.

LPA reviewed 5 staff and 2 residents files. Medications were checked and compared with doctor's orders and LIC622 Centrally Stored Medication and Destruction Records. Resident's P&I was checked and compared with last recorded balance.
