Facility Evaluation Report
On this day, July 9, 2026, at 11:25 am, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with Rachel Maniaul , assistant administrator (AADM), and Wendy Wong, licensee. LPA informed the reason for visit.
LPA toured the facility inside out with the AADM. LPA inspected the kitchen, dining room, living room, reception area, shower room with toilet, ensuite bathrooms/toilets, front, side and backyard. LPA randomly selected 10 residents rooms for inspection. Shower room was observed equipped with bathing chair, non-skid mat and grab bars. Toilets were also observed with grab bars, paper towel in dispensers for drying hands and toiletries. Food supplies were observed good for 2 days of perishables and 7 days of non-perishables. Freezers and refrigerators' temperatures were checked and observed at 0 and 40 degrees Fahrenheit respectively. Central storage for medications was locked. Storages for maintenance and gardening tools and cleaning supplies were observed locked.
Facility has smoke and carbon monoxide detectors that were tested and observed in operating condition on this day. Fire extinguishers were observed fully charge with tags showed serviced May 11, 2026. Hot water temperature in one of the ensuite toilet was tested and measured at 114.8 degrees Fahrenheit . Facility conducts drills every quarter, and records showed last conducted June 1, 2026.
