Facility Evaluation Report
At 1:15 PM on July 3, 2025, Licensing Program Analysts (LPAs) J. Sampair and A. Christy arrived unannounced to conduct an annual required inspection. LPAs met with Director Renee Tuttle, and informed the reason for visit.
LPAs toured the facility inside out with director. LPAs inspected buildings 1, 2, 3, 4, and 5 (front office building), common areas, and kitchen in building 4. Facility has adequate supplies of snacks and activity materials. First aid kits inspected and observed complete with manual. Storages for cleaning supplies were observed locked.
All buildings have fire extinguishers which LPA observed fully charged as of 11/26/2024. Hot water temperature in one of the bathrooms in building 4 was tested and measured at 110.4 degrees Fahrenheit. The emergency disaster plan last reviewed on 06/10/2025. Facility conducts disaster drills and records showed the last drill was conducted on 06/26/2025. Facility has smoke and carbon monoxide detectors that are operational. The most recent fire marshal inspection was on 1/30/2025. Facility does not handle clients money.
LPA reviewed 5 staff and 5 residents files, and interviewed 2 staff and 2 clients.
No deficiencies cited during visit. Exit interview conducted. A copy of this report was provided.
