The inspector’s account

On 10/23/2025 at 9:20 AM, Licensing Program Analyst (LPA) Luisa Fontanilla arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Licensee, Emma Sabado and Administrator, Conrad Sabado. The facility’s fire clearance was approved for 6 ambulatory only.

LPA toured the facility inside and out including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 5 total bedrooms. 3 of 5 bedrooms are occupied by the clients and 2 are occupied by staff. There are no bodies of water. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 139.2 degree Fahrenheit. All toilets, hand washing and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for clients. There is a minimum of one week supply of nonperishable and 2-day perishables food supply. Centrally stored medication were locked and inaccessible to clients.

Smoke detectors and carbon monoxide were tested and observed in operating condition. Fire extinguisher was last serviced on 02/18/25. First aid kit was observed to be complete. Fire drill was last conducted on 10/15/25.

At 10:15 AM, LPA reviewed 3 of 5 client records and 3 staff records. All staff are fingerprint cleared and associated to the facility. They have current first aid and CPR training. At 9:20 AM, LPA reviewed client's P&I money with log. LPAs reviewed clients' medications. All records were observed to be complete.
