Facility Evaluation Report
On 10/6/2023 starting at 11:15 AM, Licensing Program Analyst (LPA) L. Fici arrived unannounced to conduct 1-Year Annual Required Inspection. LPA met with Edith Sarmiento, Administrators (ADM) and explained the purpose of the visit. Administrator certificate expired on 6/12/2023, but is in process. The facility’s fire clearance is approved for six (6) non-ambulatory clients, of which one (1) may be bedridden. Upon entry, LPA observed three (3) staff and zero (0) clients present during inspection.
Starting at 11:27 AM, LPA toured facility with ADM including but not limited to six (6) bedrooms, five (5) bathrooms, kitchen, and common area. The facility consists of 6 total bedrooms which 6 bedrooms are private. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 69 Degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients'. The hot water temperature in clients’ common area bathroom was measured at 109.4 Degrees Fahrenheit. Clients’ bathrooms are equipped non-skid mats. There is a minimum of one-week supply of nonperishable and 2-day supply of perishable foods. Sharps and toxins were locked and inaccessible to clients'.
Smoke detectors and carbon monoxide detector were in operating condition during visit. Fire extinguisher was observed last serviced on 6/8/2023. First aid kit was observed to be complete.
