Facility Evaluation Report
On 10/02/2025 at 9:20 AM, Licensing Program Analysts (LPAs) P. Manalo and K. Nguyen arrived unannounced to conduct a Case Management Visit regarding an incident report that was reported to CCLD on 09/29/2025. LPA met with Program Director, Ferdylin Amsden, and explained the purpose of the visit.
The incident occured on 09/26/2025 when C1 got in possession of S1's bag that contained items including but not limited to Dramamine, keys, glasses, etc.
During the visit, S1 stated that S1 left their bag in their locker on the bottom of the cubicle where clients' belongings are placed. S1 stated that S1 had noticed that S1's bag was nowhere to be found, however, another staff member mentioned to S1 that C1 was going through something in the facility van. S1 was able to retrieve S1's bag from C1's home the following day.
S2 stated that staff will leave their items in the locker unlocked. Staff are the ones responsible for providing their own locks. However, LPAs observed that the staff lockers all did not have a lock during the visit. S2 stated that there will be a in-service on properly storage conducted on 10/03/2025.
The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiency by POC date may result in additional Civil Penalties.
