The inspector’s account

On 08/19/2025 at 8:15 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct a Case Management Visit regarding an incident report that was reported to CCLD on 08/14/2025. LPA met with Program Director, Ferdylin Amsden, and explained the purpose of the visit.

The incident occurred on 08/14/2025 when Client 1 (C1) AWOL from community outing. The incident report stated that C1 was going to the restroom and did not return. C1 was found by staff at the library near the high school and library staff contacted the police during this time.

During the visit, C1's case manager stated that C1 was using the bathroom while Staff 1 (S1) was loading up the clients' belongings to the van. Case Manager stated that C1 was not supervised by S1 during this time. There was another staff with them during the outing, however, Staff 2 (S2) was inside the van supervising the other clients. Interview with Program Director revealed that the protocol is that when a client leaves the group on outings, the client needs to be supervised within 20 feet and on eyesight level.

LPA reviewed and obtained C1's Physician Report, Individual Service Plan, Staff Contact Information. Physician Report dated 04/02/2025 showed that C1 is unable to leave the facility unassisted.

The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiency by POC date may result in additional Civil Penalties.
