Facility Evaluation Report
On 05/01/2025 at 8:35 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Program Director, Ferdylin Amsden, and explained the purpose of the visit. Day program operates from 7:30 AM to 3:30 PM.
LPA toured facility including but not limited to, Media Room/Quiet Room, Relaxation Room, Changing Room, main activity room, kitchen, bathrooms, and office space. Clients bring their own lunches. Emergency supplies, including water and snacks were observed. The hot water temperature in the client bathroom was measured at 110.07 degrees Fahrenheit. There is a comfortable room temperature of 70 degrees Fahrenheit. Incontinent clients are kept clean and dry, and the facility is free of odors. The program has a total of 13 vans used for client outings.
Fire extinguishers throughout facility were observed to be full and last serviced in 08/2025. Bay Alarm Fire Inspection was last conducted on 02/04/2026. Carbon monoxide was in operational. First aid kit was checked. Emergency disaster drills were last conducted on 04/08/2026. LPA reviewed 6 staff files. All staff have First Aid/CPR Certification and are associated with the facility. LPA reviewed 6 client files. LPA reviewed client’s medication.
