Facility Evaluation Report
On 02/03/2026 at 12:15 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct a Case Management Visit regarding an incident report that was reported to CCLD on 01/28/2026. LPA met with Program Director, Ferdylin Amsden, and explained the purpose of the visit.
Incident report revealed that C1 grabbed S1’s bag from the driver seat and consumed the medication that was in S1’s bag. S1 and S2 notified S3 of the incident in which S3 proceeded to notify C1’s family and the Administrator where C1 resides in.
During the visit, LPAs interviewed Program Director and 3 staff. LPA obtained and reviewed the documented staff statements.
Interview with S2 revealed that S2’s medication was in the bag that C1 retrieved and is unknown how many pills were consumed by the client.
The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiency by POC date may result in additional Civil Penalties.
Exit interview conducted with Program Director. Appeal Rights a copy of this report, and repeat civil penalty provided.
