This is an official report of an unannounced visit/investigation of a complaint received in our office on

11/03/2023 and conducted by Evaluator Luisa Fontanilla

COMPLAINT CONTROL NUMBER: 15-AS-20231103105951

On this day at around 9:50 am, Licensing Program Analyst (LPA) Luisa Fontanilla arrived unannounced to conduct investigation on the above allegation. LPA met with Administrator Divina Fernandez and explained the purpose of visit.

During the visit, LPA obtained staff schedule, Lic 500 and interviewed the Administrator. LPA was informed by the Administrator that the four employees mentioned in the allegation are agency caregivers. Administrator states the she was informed by the agency that all four are fingerprint cleared.

Upon verification made by LPA in Guardian, all four agency caregivers are fingerprint cleared.

This agency has investigated the complaint. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without reasonable basis. We have therefore dismissed the complaint. This is an amended copy of report issued on 11/9/2023.
