Complaint Investigation Report
It was alleged that Illegal Eviction. Emails from W1 dated 01/29/2026 revealed that C1 and W2 wanted to go back to the facility to check on C1’s room and facility staff were hesitant to open the door for C1 and W2. Interview with W2 indicated that it was agreed to have C1 stay at home with W2 until the other investigation in the facility has concluded. W2 has confirmed that there was no official eviction notice given to W2 and C1. Email with Regional Center of East Bay (RCEB) case manager revealed that C1 returned to the facility on 02/23/2026. On 02/27/2026, LPA observed that C1 was in C1’s room and interview with C1 with W2 confirmed that C1 returned to the facility on 02/23/2026.
Allegation: Staff did not prevent client in care from touching other clients inappropriately
It was alleged that Staff did not prevent client in care from touching other clients inappropriately. Emails from W1 dated 02/04/2026 and interview with C1 and W2 revealed that C1 stated that C2 has hit and touched C1 inappropriately. A review of an incident report dated 09/07/2025 showed that C1 and C2 had an altercation in which C2 attempted to hit C1, however staff intervened between the two clients and separated the two. Per incident report, C1 did not get hit. Interview with S3 revealed that another incident happened when C1 and C2 were arguing and S3 got in the middle of them. C2 punched the door but C1 said that C2 punched C1 instead. C2 stated that, “I never hit another client.” 3 of 4 staff members that were interviewed revealed that they have not witnessed any clients touching other clients inappropriately. Interview with 3 of 4 clients all stated that they have not seen any clients touch another client inappropriately.
Allegation: Staff did not provide proper supervision to clients in care
It was alleged that Staff did not provide proper supervision to clients in care. Email from W1’s dated 02/04/2026 showed that during W1's visit, S2 was observed to be eating and watching tv and S4 was just standing there until the other clients arrived from their day program.
