Facility Evaluation Report
On 10/23/2025 at 2:20 PM, Licensing Program Analyst (LPA) P.Manalo arrived unannounced to conduct case management to verify Client 1’s (C1) medication and doctor’s order. LPA met with Direct Care Staff, Bibiana Alina, and explained the purpose of the visit. Administrator, Amelia Alfonso, gave authorization for staff to sign the report.
During the visit, LPA reviewed C1's new doctor's order dated 10/03/2025 and verified the new medication prescription at the facility. LPA also observed the door bell not functioning. Staff will send proof of correction of the door bell installed by 10/24/2025.
