Facility Evaluation Report
On 10/01/2025 at 12:30 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct a Case Management Visit. LPA met with Direct Care Staff, Bibiana Alina, and explained the purpose of the visit.
While LPA was at the facility for another visit, LPA observed the following:
C1 stated that S1 is not properly providing C1 with bathing needs and making statements regarding other staff members.
Interview with S2 stated that the client makes false statements regarding new staff members.
LPA reviewed C1's Individual Service Plan dated 05/10/2023- 04/30/2024 that shows that C1 makes allegations that may or may not be fully or partially true.
LPA will request the following documents sent to CCLD by 10/08/2025:

No deficiencies cited. Exit interview conducted and a copy of this report is provided.
