The inspector’s account

On 12/10/2025 at 2:50 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct a Case Management visit to follow up on C1's wheelchair belt. LPA met with Administrator (ADM), Gina Rafael, and explained the purpose of the visit.

During the visit, LPA observed C1 on the wheelchair with a seat belt on. Interview with ADM revealed that C1 is able to take out the seat belt on their own. However, LPA observed that C1 was unable to remove the seat belt on C1's own and with staff direction.

LPA and ADM had a discussion to work with C1's RCEB case manager to find an alternative seat belt that will work with C1 and C1 is able to release it on their own. ADM will notify CCLD of the facility's plan for C1.
