Facility Evaluation Report
On 10/12/2021 Licensing Program Analyst (LPA) Allison O'Hollaren arrived unannounced for another matter. LPA met with Staff Josephine Abella. Administrator Yolanda Alatas was called.
During visit, two Residents R3 and R4 from another licensed facility have been staying at facility since 10/09/2021 temporarily due to emergency evacuation. Resident R1 is at facility and Resident R2 has been staying with family since 10/07/2021, but will return on 10/13/2021. The Oakland Regional CCL office was not notified.
The following deficiency was observed (See LIC 809D) and cited from the California Code of Regulations, Title 22 and California health and safety code. Failure to correct the deficiency may result in civil penalties.
