Facility Evaluation Report

On 01/16/2026 at 8:35 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Direct Care Staff, Jonathan Valentino and explained the purpose of the visit. Administrator, Yolanda Uy, arrived shortly after. Administrator certificate is current. The facility’s fire clearance was approved for three (3) non-ambulatory clients. Administrator stated that no client has lived in the facility since 2022. Administrator is in the process of changing the facility into an RCFE.

LPA toured the facility including inside and out but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 5 bedrooms in total, of which 3 bedrooms are for the clients and 2 bedrooms for the staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 120 degree Fahrenheit. The supply of extra hygiene's was available for clients.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 02/26/2025. LPA reviewed Administrator’s file, Emergency Disaster Plan (LIC610D), and Infection Control Plan. There are no client files for review.

LPA informed Yolanda Uy to notify CCLD if they start taking clients so that another inspection can be conducted.
