Facility Evaluation Report
On 07/22/2025 at 1:20 PM, Licensing Program Analyst (LPA) P.Manalo arrived unannounced to conduct a case management visit. LPA met with Executive Director, Beena Kumar, and explained the purpose for the visit.
While LPA P.Manalo was at the facility for a complaint investigation (15-AS-20250325125744) the following deficiency was observed.
During the complaint investigation, LPA observed that the call button response time showed there were several incidents where the residents waited for more than 30 minutes for staff assistance. Interview with staff revealed that the facility’s call button system was in disrepair and staff were unable to clear the residents’ call.
The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties.
