The inspector’s account

On 09/04/2025 at 2:25 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conducted a case management visit regarding an incident report that was reported to CCLD on 08/27/2025. LPA met with Program Director, Vivian Largusa, and explained the purpose of the visit.

The incident occurred on 08/26/2025 when Client 1 (C1) went AWOL from the facility. The incident report stated that C1 was unable to find something, got upset and ran out of the door. Staff 2 (S2) immediately followed C1 after that.

Interview with Program Director, Vivian, revealed that C1 was unable to find C1’s lunch bag and got upset. However, Staff 2 (S2) was right behind C1 when C1 walked out the facility door.

Exit interview conducted and a copy of this report was provided.
