Facility Evaluation Report
On 03/24/2026 at 10:05 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Program Director, Linmei Chiao and explained the purpose of the visit. Day program operates from 9:00 AM to 3:00 PM. There were 6 staff observed working with the 3 clients here today.
LPA toured facility including but not limited to lobby, kitchen, storage room, community room, and two bathrooms. Clients bring their own lunches and snacks. Emergency supplies of water, food, portable stove, etc. observed. The hot water temperature in the client bathroom measured at 116.4 degrees Fahrenheit. Medications are not handled by this program. Restrooms are maintained in safe and in sanitary operating condition. The facility is free of odors. Fire extinguisher was last serviced on 09/16/2025. Fire drill was last conducted on 02/27/2026. First aid kit was checked. Smoke and carbon monoxide detectors were observed and operating during the visit.
LPA reviewed 5 client and 5 staff files for sampling. 5 of 5 staff have first aid training and 5 of 5 are associated with facility.
THE FOLLOWING DEFICIENCY WAS OBSERVED DURING VISIT:
At 10:33 AM, LPA observed unlocked antibiotic ointment and scissors in the quiet room.
The Facility was cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
