Facility Evaluation Report

On 07/31/2025, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct a Post Licensing inspection at 12:00 PM. LPA met with Program Director, Linmei Chiao. Facility’s fire clearance is approved for 20 ambulatory only.

LPA toured facility including but not limited to activity room, kitchen, bathrooms, community/ quiet room, and the outside recreational area. Clients bring their own lunches and snacks. Emergency supplies, including water and snacks were observed. The hot water temperature in the client bathroom was measured 115.9 degrees Fahrenheit. Restrooms are maintained in safe and in sanitary operating conditions. Medications are not handled or dispensed by this program. Incontinent clients are kept clean and dry, and the facility is free of odors.

Fire extinguisher was last serviced on 08/13/2024. First aid kit was checked and complete. Carbon monoxide and fire alarm were observed in operating condition. Fire drill was last conducted on 07/18/2025.
