Facility Evaluation Report
On 02/18/2026 at 12:00 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Desiree Fredeluces, and explained the purpose of the visit. Administrator certificate is current.
LPA toured the facility including inside and out but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 5 total bedrooms which 4 bedrooms are occupied by the clients. A comfortable temperature for clients is maintained at 72 degree Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 114.4 degree Fahrenheit. All toilets, hand washing and bathing are safe, sanitary and in operating condition. The supply of extra hygiene's was available for clients. There is a minimum of one week supply of nonperishable and 2-day perishables food supply.
Carbon monoxide were in operating condition during visit. Smoke alarm inspection was last conducted on 02/12/2026. Fire extinguisher was last serviced on 12/12/2025. First aid kit was observed to be complete. Fire drill was last conducted on 01/10/2026.
At 12:15 PM, 4 of clients’ records were reviewed. At 12:54 PM, 4 staff records were reviewed. 4 of 4 have current first aid training and associated to the facility. LPA reviewed client's P&I money with log. LPA reviewed a sample of client's medications.
