The inspector’s account

On 10/29/2025 at 2:15 PM, Licensing Program Analyst (LPA) P.Manalo arrived unannounced to conduct a case management to follow up on the Department of Disability Services (DDS) semi annual review. LPA met with House Manager, Cherry Sibal, and explained the purpose of the visit. Administrator came shortly after.

LPA conducted this visit as a follow up of the visit conducted by the DDS Nurse Consultant on August 08, 2025.

During the visit, LPA conducted interviews and obtained the following documents such as C1's Control Drug Report dated for August 2025. LPA reviewed the Centrally Stored Medication for C1 and C2.
