Facility Evaluation Report
On 04/17/2025 at 10:40 AM, Licensing Program Analyst (LPA) P. Manalo conducted an unannounced case management visit regarding an incident report that was reported to CCLD on 04/01/2025. LPA met with Administrator, Desiree Fredeluces, and explained the purpose of the visit.
The incident occurred on 03/25/2025 when Client 1 (C1) contacted the Administrator that Staff 1 (S1) did not treat C1 with respect and dignity.
LPA interviewed Administrator, Staff (1), and C1. Interviews with C1 indicated that C1 likes living here and that staff treats C1 well. C1 does not remember the incident.
LPA obtained the 4 training logs that was conducted after the incident and the coaching record for S1.
Administrator conducted a training with S1 on personal rights of client's on 04/02/2025 as a reminder for staff to treat clients with respect.
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
