Facility Evaluation Report
On 06/02/2026 at 12:00 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with the Program Director, Elizabeth De La Torre and explained the purpose of the visit. Day program operates from 8:00 AM to 3:00 PM. The facility capacity is 51 clients, and 20 clients may be non-ambulatory.
During the visit, LPA inspected the Adult Day Program (ADP) including but not limited to multiple bathrooms, activity rooms such as Training Room, Academic Classroom, Sensory Room, Game Room, and Recreational area. Hot water temperature was measured at 107.7 degrees Fahrenheit in one of the clients’ bathroom. There was multiple fire extinguishers observed and last serviced on 08/27/2025. Room temperature was observed at 70 degrees Fahrenheit.
LPA reviewed 6 staff and 6 client files. All staff are fingerprint cleared and associated to the ADP. Fire drill was last conducted on 05/29/2026. First Aid kit was observed to be complete. Fire alarm and carbon monoxide were tested and are in operating condition during visit.
