Facility Evaluation Report
On 10/03/2025 at 11:05 AM, Licensing Program Analysts (LPAs) K. Nguyen and P. Manalo conducted an unannounced case management visit regarding an incident report that was reported to CCLD on 09/29/2025. LPAs met with Administrator, Monique Tran, and explained the purpose of the visit.
The incident occurred on 09/28/2025 when Administrator was notified by another staff member that Client 1 (C1) had a bruise on C1's thigh.
During the visit, Administrator stated that during an outing, staff notified Administrator that the wheelchair tipped causing C1's bruising. However, C1 did not fall out of the wheelchair.
Exit interview conducted and a copy of this report provided.
