Facility Evaluation Report
On 01/14/2026 at 4:05 PM, Licensing Program Analysts (LPAs) P.Manalo and K. Nguyen arrived unannounced to conduct a case management visit. LPA met with Executive Director, Gianni Amari. observed during the annual inspection that the facility is over capacity. The facility capacity is 128 and the facility census is 130 residents.
While LPAs were at the facility for another visit, LPAs observed the following deficiency:

The deficiency was observed (see LIC 809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiency may result in civil penalties.
