Facility Evaluation Report
On 04/22/2026 at 9:25 AM, Licensing Program Analyst (LPA) Patricia Manalo and Investigator, Eddie Phung, arrived announced to conduct a closure inspection visit. LPA met with Licensee, David Bloch and staff, Natalia Zuberbuhler and explained the purpose of the visit.
On 04/02/2026, Licensee sent LPA P.Manalo a formal notice to the Department that the Adult Day Program will be closing effective 04/01/2026 due to the facility no longer vendorized by Regional Center of East Bay (RCEB) as of 03/26/2026.
During the visit, LPA, Investigator, and facility staff toured the facility including but not limited to, activity room, kitchen, bathroom, and the outside recreational area. LPA observed no clients present. Zuberbuhler stated that the facility notified the clients' families that the facility was closing on 03/26/2026.
LPA obtained the original license and will send forfeiture letter at a later time.
