On 03/18/2026 at 3:20 PM, Licensing Program Analysts (LPAs) P. Manalo and K. Nguyen arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. LPAs met with Licensee, David Bloch, and explained the purpose of the visit.
During visit, LPAs hand delivered the immediate exclusion letter for S1 to Administrator. Licensee states that S1 is not currently employed at the facility and will be immediately removed from the facility's Guardian Roster.
No deficiencies are being cited on this date.
