Facility Evaluation Report
On 11/18/2025 at 1:45 PM, Licensing Program Analyst (LPA) P.Manalo conducted an unannounced Case Management visit regarding a self-reported incident that was reported on 07/28/2025 and to follow up on C1's Fall Prevention Plan. LPA met with Administrator, Amelia Alfonso, and explained the purpose of the visit.
On 07/28/2025, LPA P. Manalo received an incident report that C1 had a fall and sustained bruises on C1's hand and face. Staff attended to C1's needs as soon as the fall occured. However, interview with Administrator revealed that C1 does not have a 1:1 care. Incident report indicated that the facility took in preventative measures by notifying C1's doctor's of the incident to obtain a follow up appointment and had C1's medication order changed to address the falls.
LPA is requesting the facility to send Health Care Contact Record dated 05/16/2025, 05/29/2025, 08/08/2025, and 09/05/2025 along with the after-visit attached to the document.
