Facility Evaluation Report
On 08/18/2025 at 2:00 PM, Licensing Program Analysts (LPAs) P.Manalo and K.Nguyen arrived unannounced to conduct a case management visit. LPA met with General Manager, Ryan Turner, and explained the purpose of the visit.
While LPAs was at the facility for another visit, LPAs observed the following deficiency:
LPAs observed Tide Pod in the washing machine room in Memory Care 2.
LPAs observed Sanitation Wipes unlocked in Memory Care 1.
The deficiencies were observed (see LIC 809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiency may result in civil penalties.
