Facility Evaluation Report
On 4/6/2023 at 9:50AM, Licensing Program Analysts (LPAs) Kelly Nguyen and Grace Luk conducted a case management visit. While at this facility for a POC visit, LPAs observed the following deficiency.
At 9:24AM, LPAs observed that staff member (S1) is not associated to this facility. LPAs verified that S1 is fingerprint cleared but not associated. S1 has worked at this facility on and off since 2019. Administrator associated S1 during visit.
The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiency by POC date may result in additional Civil Penalties.
