On 9/14/2022 at 1:20PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a case management visit. LPA met with caregiver, Ofelia Calderon. Licensee, Merdith Castro arrived 2 hours later.
LPA is conducting a case management visit as a result of audit documents not received by due date.
The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties.
