This is an official report of an unannounced visit/investigation of a complaint received in our office on

03/09/2022 and conducted by Evaluator Grace Luk

COMPLAINT CONTROL NUMBER: 15-AS-20220309090421

On 11/18/2022 at 12:20PM, Licensing Program Analysts (LPAs) G. Luk and K. Nguyen arrived unannounced to deliver findings in regards to the allegation above. LPAs met with caregiver, Lester Calma.

During the course of investigation, LPA G. Luk interviewed resident, staff, and complainant. LPA reviewed and obtained documents including audit report.

Interview with resident revealed that staff are providing care and supervision to residents. Interview with staff indicated there were no issues with utility bills and staff are receiving their paychecks.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is UNSUBSTANTIATED. Exit interview conducted. A copy of this report provided.
