Complaint Investigation Report
It was alleged staff used inappropriate language towards a client. Based on information obtained, staff cursed at client. However, LPAs interviewed 2 clients and 2 of 2 denied staff cursing at them. LPA interviewed 3 staff of which 3 of 3 denied the allegation.
It was alleged staff pinched a client while in care. Based on interview with C1, C1 witnessed S1 pinched C2. However, interview with S1 and S2 revealed that S1 does not provide direct care for C2.
It was alleged staff hit a client while in care. Based on information obtained, staff kicked client on the stomach. During an interview with C1, she stated that she did not report incident during the time it occurred and there were no witnesses. Interview with 3 staff revealed they were not aware of C1 being kicked by staff.
Although the allegations may have happened or are valid, there are not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED .
