Facility Evaluation Report
On 05/01/2025 at 9:10 AM, Licensing Program Analyst (LPA) Ardalan Gharachorloo arrived unannounced to conduct a case management visit regarding the incident report received on 04/30/2025. LPA met with Administrator, Xiang (David) Lin and explained the purpose of the visit.
LPA received an incident report from the facility stating that as of 05/01/2025, R1 has not returned to the facility since 04/22/2025.
During the visit, LPA interviewed the Administrator, who stated that the responsible party and police are aware of and are investigating the incident. The Administrator also stated that he is continuously updating the LPA regarding this incident and, according to the responsible party, R1 is currently safe. The Administrator further stated, "within the past 12 months, this is the second time this incident has happened".
The Administrator stated that he will continue updating the LPA about this incident and will be sending a new incident report. LPA reviewed R1s file during the visit.
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
