On 12/23/2024 at 8:30 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection of the facility. Upon arrival, LPA stated the purpose of the visit.
After the LPA inspected the facility inside and outside, he realized that this was the wrong facility, so the Required Annual Inspection was halted.
During the inspection, the LPA discovered 1 staff member and 1 family member of staff member were not associated with the facility
