The inspector’s account

Licensing Program Analyst (LPA) Ardalan Gharachorloo conducted a Case Management visit as a result of licensee requested closure of the facility. Upon arrival at 9:50 AM, LPA met with Licensee, George Druga and explained the purpose of the visit.

On 07/01/2025, Licensee sent the LPA a letter requesting to surrender the license due to retirement.

Starting at 10:20 AM, LPA toured entire facility with Licensee including kitchen, bathrooms, bedrooms, common areas, backyard and garage. LPA confirmed all residents have moved out. Licensee surrendered his facility license during today's visit.

A forfeiture letter will be mailed to licensee at a later time. Exit interview conducted and a copy of this report provided.
