Complaint Investigation Report

LPA observed that R1 was re-evaluated in December of 2021 after the incident. S2 stated options were discussed with R1's family to prevent future incident and staff are always observing residents to see if assistance is needed.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is UNSUBSTANTIATED .
