Facility Evaluation Report

Licensing Program Analyst (LPA) Hiratsuka, conducted this unannounced annual visit. LPA met with Licensee Jacob Meehan. Administrator Concepcion Targa arrived during the visit.

This facility has a fire clearance for three ambulatory and one non-ambulatory for a total of four residents. This facility has four private resident rooms and one staff room. There are two full common bathroom and the staff room has a full private bathroom. Food supply meets regulations.

Three resident files and one staff file were reviewed.
