Facility Evaluation Report

Licensing Program Analyst Hiratsuka, conducted this unannounced annual inspection. LPA met with Administrator Michael Warner, Licensee Kathleen Warner, and Employees Sandra Cochran and Diana Kuhn..

LPA toured the facility with Mr. Warner including common areas, kitchen, bedrooms, hallways, storage, outside, patio areas, pool area with locked fence, storage sheds, etc.

The home is clean and well-furnished, plenty of food supplies.

Resident rooms have required furnishings and private client belongings.

LPA reviewed staff files and resident files, Files included all required documents.

At this time, the facility appears to be in substantial compliance with regulations. No deficiencies were cited at this visit.
