The inspector’s account

Licensing Program Analyst Hiratsuka, conducted this unannounced case management visit in response to two incident reports submitted by Administrator Benita Valenzuela-Lopez. Administrator had issues submitting the reports to Community Care Licensing Division.

The first incident was on 10/03/2025, when a resident was able to access the medication cabinet and take some of their pills. The resident was taken to the hospital and suffered no ill-effects. The resident appears to have found the magnet fob that unlocks the medication cabinet because the magnet fob was stored just below the cabinet. The resident did not destroy the cabinet or lock. Medications have since been moved to a locked room.

The second incident occurred on 10/08/2025, and a caregiver forgot to give the resident an evening medication. The resident's doctor was contacted and the resident did not suffer any ill-effects.

Based on the above the following deficiencies have been cited.

Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Appeal rights were provided.
