Licensing Program Analyst (LPA) Hiratsuka conducted this unannounced annual visit. LPA met with Caregiver Romulo Agustino

This facility has a fire clearance for three ambulatory and one non-ambulatory. This facility has a four private resident rooms. None of the resident rooms have exits to the outside. There is a staff room that has an exit to the outside and a full private bathroom. There is one full common bathroom. There are two sitting areas and kitchen. There is a door leading to the garage from the laundry room. The backyard was inspected. There is a gate on the same side as the garage.
