Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management inspection. LPA met with Karen Warner during today's inspection. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: Masks.

During today's inspection LPA were following up on an Alta regional center visit conducted in February 2023. LPA reviewed C1's IPP, LIC602, and needs and service plan. C1 is diabetic and independent with their insulin injections and testing. Training is being provided to staff and proper documentation is being recorded.

No deficiencies cited during today's inspection.
