Facility Evaluation Report

Licensing Program Analyst (LPA) Hiratsuka arrived at the facility unannounced to conduct an annual inspection. LPA met with Clifford Lelina and Ayson Aguirre. No residents were present during this visit because they were all at programs.

This facility has a fire clearance for three ambulatory and one non-ambulatory residents. The facility has four private resident rooms. There is one staff room that has a full private bathroom. There is one full common bathroom. There is an ample supply of perishable and nonperishable food. In the areas toured no immediate health, safety, or personal rights violations were observed. Staff and resident files were reviewed. Medications were also reviewed.

Two resident files were reviewed and three staff files were reviewed.
