Facility Evaluation Report

The purpose of this informal office meeting is a follow-up to a meeting that was conducted on May 27, 2025.. This meeting was conducted via Microsoft Teams. Attending this meeting representing Community Care Licensing Division 9CCLD) was Licensing Program Analyst Kerry Hiratsuka and Licensing Program Manager Troy Ordonez. Representing Alta California Regional Center (ACRC) was Community Services Specialist Tom Kasper; Leed Community Services Specialist Jason Scantlebury; Quality Assurance Specialist Kris Takeda-Miller, and Quality Assurance Manager Scott Barr,. Representing the facility is Licensee Jennifer Esterline.

During this meeting Licensee stated she is closing the program due to issues with operations. She stated she will give notice to the clients, their representatives, She stated she is gong to work to ensure the closure follows the regulations for both CCLD and ACRC.

A hard copy of this report is going to be emailed to Licensee for her signature and Licensee is to email a signed copy back to LPA Hiratsuka.
