Facility Evaluation Report
The purpose of this informal office meeting is to address the facility’s repeated noncompliance and to discuss a corrective action plan to ensure the facility meets California Code of Regulations, Title 22 requirements. This meeting was conducted via Microsoft Teams. Attending this meeting representing Community Care Licensing Division was Licensing Program Analyst Kerry Hiratsuka and Licensing Program Manager Troy Ordonez. Representing Alta California Regional Center was Community Services Specialist Tom Kasper; Leed Community Services Specialist Jason Scantlebury; and Quality Assurance Specialist Yvette Beltran. Representing the facility is Licensee Jennifer Esterline.
The Department called this meeting to provide CEO, with the opportunity to correct the deficiencies addressed during the office meeting. During the office meeting the following issues were discussed:
Compliance History- The facility has a history of repeated deficiencies, including but not limited to:
• 12/06/2023: Substantiated complaint for facility disrepair and lack of qualified director. Two Type B citations issued.
• 05/14/2025: Substantiated complaint for facility disrepair and lack of administrator. Two Type B citations issued.
• Patterns noted: structural deterioration, sanitation concerns, absent administrative oversight, and delayed Department communication.
